Bounce Sheffield waiver terms and conditions I wish the adult/child/children to participate in indoor trampoline activities including, but not limited to trampolining, trampoline park access, café access, (collectively hereinafter called “the Activity”) organised by Bounce Sheffield. I declare that I am the parent or legal guardian of the child/children or I have authority from the child’s parent or legal guardian to sign this Waiver Form. I agree that the adult/child/chidren will carry out the activity in accordance with the specific safety instructions that will be given before undertaking the activity. I agree that the adult/child/chidren will wear any safety equipment provided by Bounce Sheffield Trampolining in accordance with the safety instructions and that he/she will follow the directives of Bounce Trampolining, its management and staff at all times. I confirm that the adult/child/children is in good physical condition and has no medical impairment that might prevent him/her from the intended use of Bounce Trampolining. I certify that to the best of my knowledge the adult/child/chidren listed is/are not pregnant. I acknowledge that the maximum permitted weight for participation in the Activities is 18 stone and confirm that the adult/child/children listed do not exceed this weight. I acknowledge that Bounce Sheffield Trampolining did not give me medical advice relating to the adutl/child’s physical condition and ability to use the facilities. I acknowledge that the Activity is potentially dangerous and that by participating in the Activity the adutl/child/chidlren will be exposed to the possibility of personal injury. I acknowledge that I am responsible for the safety & supervision of the adutl/child/chidren listed (and the safety of our possessions) and that Bounce Sheffield do not provide supervision of adutls/children whilst using the premises or adutlts/children attending unaccompanied or if left unaccompanied on the premises. I hereby indemnify Bounce Trampolining against all claims made by any other person against Bounce Trampolining in respect of any injury, loss or damage arising out of or in connection with the failure to comply with the safety instructions and /or directions of Bounce Sheffield Trampolining, its management or staff.
